This does not mean the symptoms are imagined. Instead, stress may influence digestive movement, sensitivity and the way the brain interprets sensations coming from the gut.
Imagine your nervous system becoming more alert before:
an important meeting
travelling
eating at a restaurant
wearing fitted clothing
attending an event
being somewhere without easy bathroom access
If bloating has been uncomfortable or embarrassing in similar situations before, your brain may start paying much more attention to your abdomen.
Small digestive sensations may become highly noticeable.
Your nervous system may become increasingly alert to sensations from the gut.
And the more closely you monitor the gut, the easier it may become to notice every sensation occurring there. This can contribute to a gut–brain loop.
For some people, bloating anxiety can gradually develop around the anticipation of symptoms. It may look something like this:
A digestive sensation appears

You notice your abdomen
↓
“What if I get bloated?”

Your attention becomes increasingly focused on your gut
↓
The nervous system becomes more alert
↓
Digestive sensations feel stronger
↓
The bloating becomes more concerning
↓
You monitor the abdomen even more closely
This does not mean anxiety created an imaginary digestive symptom. There may be digestive processes occurring at the same time.
The important point is that the gut, brain and nervous system can influence each other.
For some people, the hardest part of IBS bloating is not necessarily the physical sensation itself. It is everything organised around trying to prevent it.
You might start:
choosing loose clothing because you expect your abdomen to change
skipping meals before social events
avoiding particular restaurants
repeatedly checking how your stomach looks or feels
planning your day around when you usually become bloated
cancelling plans because you are worried about how your body will feel
feeling uncomfortable being photographed later in the day
becoming anxious before eating something unfamiliar
Eventually, the anticipation of bloating can become almost as disruptive as the bloating itself. Your day may gradually become organised around trying to keep your gut predictable.
Avoidance makes sense when the brain believes it is protecting you.
If a particular meal, outfit or social event previously occurred alongside severe bloating, your nervous system may remember that experience. The next time something similar appears, your brain may respond early.
For example: “Last time I ate before going out, I became extremely bloated.”
may gradually become: “Eating before going out is risky.”
Then: “Going out when I have eaten feels unsafe.”
The behaviour is protective. But repeatedly avoiding these situations can strengthen the association between the situation, the symptom and a sense of threat.
