Stress-related symptoms do not always begin with a deliberate anxious thought.
The nervous system constantly uses past experiences, current sensations and the environment to predict what may happen next.
For example, someone who previously experienced severe urgency in traffic may begin reacting as soon as they see congestion ahead.
They may not be consciously thinking, “This is dangerous.”
Their brain may already have connected traffic, limited escape and uncertain bathroom access with risk.
The body then responds protectively. Breathing changes, muscles tighten, attention moves towards the abdomen and bowel activity may increase.
This is a learned biological response, not an imagined one.
The gut microbiome is also part of the gut–brain conversation.
The trillions of microorganisms living in the gut help support digestion, immune function and the signals travelling between the gut and brain.
When the body is under ongoing stress, the environment in the gut can change too. In turn, changes in the gut may influence how settled, sensitive or reactive the whole system feels.
The microbiome is only one part of the picture, but it reminds us that digestion, stress, immunity and the nervous system are closely connected.
For some people, IBS begins after gastroenteritis, food poisoning or traveller’s diarrhoea. This is known as post-infectious IBS.
The original infection may clear, but changes in gut sensitivity, immune activity, bowel movement or the gut environment may continue.
Stress may then interact with this sensitised system and contribute to ongoing symptoms.
This does not mean stress caused the infection or the IBS. It means infection, immune activity, gut sensitivity and stress may influence one another.
Sometimes there is a clear starting point, such as an infection, prolonged stress, a painful digestive episode, disrupted sleep or a frightening experience of urgency.
Symptoms may continue after the original trigger has passed because gut nerves remain sensitive, bowel patterns stay altered or the nervous system continues predicting that symptoms will return.
Fear, monitoring and avoidance can then reinforce the protective response.

This is sometimes called body scanning or hypervigilance.

The person is not choosing this response. They are caught in a protective loop.
There is no single cause of IBS.

Different factors may matter for different people, and several may overlap.

A protective response may have developed for understandable reasons, but it does not always need to remain at the same intensity.
